What Is an Immigration Medical Exam?

An immigration medical exam is a required health screening for most applicants seeking adjustment of status to lawful permanent resident (green card). The examination must be performed by a USCIS-designated civil surgeon and documented on Form I-693, Report of Immigration Medical Examination and Vaccination Record. The purpose is to determine whether you have any health-related conditions that would make you inadmissible to the United States under immigration law.

How Long Is Form I-693 Valid?

USCIS updated its policy on Form I-693 validity effective June 11, 2025. Under the current rules:

Forms signed on or after November 1, 2023: Valid only while the specific application it was submitted with is pending. If your application is denied or withdrawn, that Form I-693 is no longer valid, and you will need a new medical exam for any future application.
Forms signed before November 1, 2023: Valid for two years from the date of the civil surgeon’s signature.

This policy change means you cannot reuse a previous Form I-693 if your earlier application was denied or withdrawn. Plan to complete your immigration medical exam close to when you file your Form I-485.

What to Expect During Your Immigration Medical Exam

The immigration medical exam includes several components:

  • Medical history review: Discussion of your health history, including any chronic conditions or past surgeries.
  • Physical examination: A complete physical exam to assess your general health.
  • Vaccination review: Verification of required immunizations and administration of any vaccines you need.
  • Laboratory testing: Blood draw and other tests as required, including screening for syphilis and tuberculosis.
  • Form completion: Once all results are received, we complete and seal your Form I-693 for submission to USCIS.

What to Bring to Your Appointment

Please bring the following documents to your immigration medical exam appointment:

  • Government-issued photo identification (passport, state ID, or driver’s license)
  • Vaccination records showing all immunizations you have received
  • Medical records for any chronic conditions
  • Payment for the examination (immigration medical exams are typically not covered by insurance)

If you have questions about the required documentation, please reach out to our office to speak with a helpful and knowledgeable staff member.

Required Vaccinations for Immigration

USCIS requires applicants to show proof of vaccination against certain diseases. The specific vaccines required depend on your age and are based on recommendations from the Centers for Disease Control and Prevention (CDC). Common required vaccines include:

  • Measles, Mumps, and Rubella (MMR)
  • Tetanus and Diphtheria (Td or Tdap)
  • Hepatitis B
  • Varicella (Chickenpox)
  • Influenza (seasonal)
  • Pneumococcal (for certain age groups)

If you do not have vaccination records, we can perform blood tests (titer tests) to check your immunity. Based on the results, we will administer any necessary vaccines.

Frequently Asked Questions

How much does the immigration medical exam cost?

The cost varies depending on which vaccinations and tests you need. Immigration medical exams are typically not covered by health insurance. Contact our office for current pricing information.

How long does the immigration medical exam take?

The initial appointment typically takes one to two hours, including the physical exam, blood draw, and vaccinations. You may need to return for a follow-up visit once lab results are ready, at which point we complete and seal your Form I-693.

What if my previous green card application was denied?

Under USCIS policy effective June 2025, if your previous application was denied or withdrawn, your Form I-693 from that application is no longer valid. You will need to complete a new immigration medical exam and submit a new Form I-693 with your new application.

What happens if I do not have my vaccination records?

If you cannot provide vaccination records, our civil surgeon can order blood tests (titer tests) to determine your immunity to certain diseases. Based on the results, we will administer any vaccinations you need to meet USCIS requirements.

Do you accept insurance for immigration medical exams?

Immigration medical exams are typically not covered by insurance. However, we accept various payment methods. Visit our accepted insurance page for more information about coverage for other services.

Can I submit Form I-693 separately from my Form I-485?

Since December 2024, USCIS requires applicants to submit Form I-693 together with Form I-485 (Application to Register Permanent Residence or Adjust Status). Submitting them together helps avoid delays and requests for additional evidence.

What is a civil surgeon?

A civil surgeon is a medical doctor designated by USCIS to perform immigration medical examinations. Only designated civil surgeons can complete Form I-693. You can verify a provider’s designation using the USCIS civil surgeon finder tool.
